JOB SUMMARY

 

The Case Manager supports daily program operations and collaborates to ensure timely completion of comprehensive evaluations.  This includes assisting with obtaining releases, requesting and picking up documents from other providers, transporting documents to and from the court, coordinating transportation and/or transporting clients to and from appointments with other IE staff; assisting with in-home family assessments as needed; administering screening tools as appropriate to assist in data collection.

ESSENTIAL DUTIES AND RESPONSIBILITIES

 

Facilitates rapid attainment of releases, records, external and internal communications and background information; goes out to obtain and deliver records from other providers and from/to the courts.

 

 

 

Provides supervision of clients as needed to keep clients safe, supported and to provide observations of behavioral interactions.

 

 

 

Assists in the identification of community resources for clients.

 

 

 

Maintains records of case management activities and communications for reporting and tracking purposes and completes all documentation for client record in accordance with documentation timelines and in compliance with agency and program standards.

 

 

 

Attends team meetings, staff meetings or rounds as needed to assist in coordinating case management and to provide feedback regarding interactions with clients and families.

 

 

 

Supports daily program specific operational functions as required. May transport clients to appointments or linkages as appropriate.

 

 

 

Performs job duties in the office and/or community settings outside of the office (e.g., homes, schools, hospitals, institutions, other community locations) as defined by the program.

 

 

 

Assists in the administration of standardized screening tools (as appropriate based on training and required credentials) and collects data as defined by program.

 

 

 

Assists other staff, where indicated, with home visits and family assessments.  For bilingual case managers, this may include providing interpretation/translation.

 

 

 

Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.

 Requirements

REQUIREMENTS/EDUCATION AND EXPERIENCE/QUALIFICATIONS

BA/BS degree in related field plus strong organization and communication skills, with evidence of ability to work with a team is required.  Displays excellent work habits (attendance, timeliness, professionalism), is open to feedback, and has good time management skills.

Current program needs require the ability to fluently speak English and Spanish.
